Care Guide

Skilled Nursing

Skilled nursing provides 24-hour medical and nursing care, plus short-term rehabilitation, for seniors with serious health needs. In the East Valley, long-term nursing care typically costs $8,000 to $10,000+ per month. This guide explains what it covers and how it differs from assisted living.

Best Suited For:

Seniors who need 24-hour licensed nursing care or short-term rehabilitation after a hospital stay, or who have complex medical needs beyond what assisted living or home care can safely provide.

What Is Skilled Nursing?

Skilled nursing care is the highest level of daily medical care outside of a hospital. A skilled nursing facility (often called a nursing home) provides 24-hour licensed nursing, help with all daily activities, and access to therapies such as physical, occupational, and speech therapy. There are two common reasons a senior enters skilled nursing:

  • Short-term rehabilitation after a hospital stay (for example, recovering from a stroke, surgery, or a serious fall).
  • Long-term care for those with complex, ongoing medical needs that cannot be met in assisted living or at home.

In Arizona, skilled nursing facilities are federally regulated and state-licensed, distinct from AZDHS-licensed assisted living.

Who It's For

Skilled nursing is appropriate for seniors who:

  • Need daily licensed nursing care or complex medical management.
  • Are recovering from surgery, a stroke, or a serious illness and need rehab.
  • Have advanced conditions that assisted living or home care cannot safely handle.
  • Require care that goes beyond what a non-medical setting can provide.

Many seniors do not need skilled nursing long-term — assisted living or memory care meets most needs. Skilled nursing is for genuine medical complexity.

What It Costs in the East Valley

Skilled nursing is the most expensive level of care. Long-term skilled nursing in the East Valley typically runs $8,000 to $10,000 or more per month (often quoted as a daily rate). Payment differs from assisted living:

  • Medicare may cover short-term rehab in a skilled nursing facility for a limited time (up to 100 days per benefit period, with cost-sharing after day 20) when it follows a qualifying hospital stay.
  • ALTCS — Arizona's Medicaid long-term care program — can cover long-term skilled nursing for those who qualify.
  • Private pay and long-term care insurance for those who do not qualify for the above.

Skilled Nursing vs. Alternatives

  • Skilled nursing vs. assisted living: Assisted living is non-medical daily support; skilled nursing provides 24-hour licensed nursing and rehab. See our FAQ on the difference.
  • Skilled nursing vs. memory care: Memory care focuses on dementia supervision; skilled nursing focuses on medical needs, though some residents need both.
  • Skilled nursing vs. in-home care: Home care is non-medical; skilled nursing delivers hospital-adjacent medical care.

Questions to Ask When Touring

  • Is this stay for short-term rehab or long-term care, and how is each paid for?
  • What is the nurse and CNA staffing ratio, day and night?
  • What is the facility's most recent Medicare star rating and inspection history?
  • What therapies are offered on-site, and how often?
  • Do you accept ALTCS for long-term stays?
